Parent, Teacher, Student Survey
In an effort to approve our school the district is asking that all staff, parents, and students fill out their respective survey found at:
If a staff member is working in
a remote location that is not part of the District network, they will be prompted to enter a password. Entering the password, ccsdremotestaff2018, will enable access to the survey.
After accessing the Staff
Survey, each staff member must type in his/her location code to complete the survey.
After accessing the Student Survey, each student must type his/her student ID number and birthdate, select the preferred language (English or Spanish), and complete the survey.
If a parent/guardian requests a paper survey from a school, contact the Research Department at (702) 799-1041, option 4, or e-mail
firstname.lastname@example.org. All requests must be made by December 14, 2018.
