DMV Forms

Processing DMV forms will continue through the Attendance office.

Please allow 24-48 hours for the DMV form to be completed.

The DMV form is valid for 60 days.

Students may access https://dmvnv.com/nvdlteens.htm to request a DMV-301 form.

Students, please remember………………the DMV form will NOT be issued to

you if you have more than 8 unexcused absences for the last semester.

Please check your absences before putting in the request for the DMV

form.

You will need both your attendance report and the DMV form in order to

get your permit/license.
